By accessing, scheduling, or using this website and related services, you agree to be bound by these Terms. If you do not agree, you must discontinue use immediately.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">These Terms apply to all users, including prospective patients, counseling clients, website visitors, and contributors of content.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">These Terms are governed by, and services under this Agreement are provided by, Jose Gonzalez-Soto Psychiatric Mental Health Nurse Practitioner, PLLC. The Practice serves clients located in Florida and New York only. Depending on where you are located and receive services:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>Florida clients<\/strong> are served by Jose Gonzalez-Soto Psychiatric Mental Health Nurse Practitioner, PLLC, doing business as <strong>&#8220;Reintegrate Mental Health &amp; Counseling&#8221;<\/strong>, a registered Florida fictitious name (d\/b\/a) of the Practice. References to &#8220;ReIntegrate,&#8221; &#8220;we,&#8221; &#8220;our,&#8221; or &#8220;us&#8221; as applied to Florida clients refer to the Practice operating under this trade name.<\/li>\n\n\n\n<li><strong>New York clients<\/strong> are served directly by Jose Gonzalez-Soto Psychiatric Mental Health Nurse Practitioner, PLLC, under its legal name. The &#8220;Reintegrate&#8221; trade name is not used in connection with services provided to New York clients.<\/li>\n<\/ul>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h2 class=\"wp-block-heading\">1. Scope of Services and Disclaimer<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">The Practice provides <strong>virtual psychiatric and\/or counseling services<\/strong> in compliance with state licensure requirements:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>New York and Florida:<\/strong> Virtual psychiatric services (including evaluation, diagnosis, treatment, and medication management) and counseling services.<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">All clinical services are delivered exclusively by licensed professionals, including but not limited to a board-certified Psychiatric Mental Health Nurse Practitioner (PMHNP-BC), Advanced Practice Addiction Registered Nurse (CARN-AP), and a Licensed Mental Health Counselor (LMHC).<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">You acknowledge and agree:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Website content, blog posts, and educational materials are provided for <strong>informational purposes only<\/strong>. They do <strong>not<\/strong> create a provider-patient or counselor-client relationship.<\/li>\n\n\n\n<li>A professional relationship with the Practice is established <strong>only after<\/strong> intake, informed consent, verification of eligibility, and execution of required agreements.<\/li>\n\n\n\n<li>Information on the website should never replace medical, psychiatric, or counseling advice.<\/li>\n\n\n\n<li>The Practice <strong>does not<\/strong> provide emergency services. <strong>In the event of an emergency, you agree to immediately contact 911 or go to the nearest emergency room. For suicidal or homicidal thoughts, contact 988.<\/strong> Reportable risks and incidents will be managed according to federal and state law.<\/li>\n<\/ul>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h2 class=\"wp-block-heading\">2. Eligibility and Jurisdiction<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">By engaging services, you represent and warrant that:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>You are <strong>at least 18<\/strong> years old or the legal age of majority in your jurisdiction.<\/li>\n\n\n\n<li>For clients <strong>under 18<\/strong>, consent for services must be provided by a parent or legal guardian, except when otherwise authorized by law.<\/li>\n\n\n\n<li>You are <strong>physically located in New York or Florida<\/strong> when receiving services.<\/li>\n\n\n\n<li>You will not attempt to access services if you are located in a jurisdiction where the Practice is not licensed or legally permitted to practice.<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">We reserve the right to refuse or discontinue services if legal, ethical, or clinical standards cannot be met.<\/p>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h2 class=\"wp-block-heading\">3. Consent to Telehealth Services<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Prior to the commencement of services, clients will receive and sign an informed consent agreement detailing the scope, risks, benefits, and limitations of treatment, and the nature of telehealth services. Consent may be withdrawn at any time, subject to safe discontinuation of care. Your attendance at the initial session constitutes acceptance of these Terms. By requesting services, you consent to the use of <strong>telehealth technologies<\/strong> (video, phone, or other secure platforms). You understand and accept:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Telehealth may carry risks, including technical interruptions, security limitations, or incomplete clinical information.<\/li>\n\n\n\n<li>You must provide accurate identifying and health information.<\/li>\n\n\n\n<li>You are responsible for ensuring a private and safe environment during sessions.<\/li>\n\n\n\n<li>You may withdraw consent to telehealth at any time, recognizing that this may limit service availability.<\/li>\n<\/ul>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h2 class=\"wp-block-heading\">4. Fees, Billing, Insurance and Cancellations<\/h2>\n\n\n\n<ul class=\"wp-block-list\">\n<li>You agree to provide complete and accurate billing and insurance information.<\/li>\n\n\n\n<li>Fees for services are subject to change; you will be notified in advance.\n<ul class=\"wp-block-list\">\n<li>$250 per 60-minute Initial Mental Health Evaluation<\/li>\n\n\n\n<li>$150 per 30-minute Medication Management with Counseling Session<\/li>\n<\/ul>\n<\/li>\n\n\n\n<li>Unless otherwise stated, payment is due at the time of service. We accept cash, health savings account (HSA) cards, and all major credit cards.<\/li>\n\n\n\n<li>Cancellations must be received at least <strong>48 hours in advance<\/strong> to avoid a late or no-show fee of $95.<\/li>\n\n\n\n<li>Insurance claims will be submitted only if the Practice is credentialed with your plan. Otherwise, services are self-pay.<\/li>\n\n\n\n<li>Depending on your health insurance provider or employee benefit plan, services may be covered in full or in part. Please contact your insurance provider to verify how your plan compensates you for out-of-network psychiatric and counseling services.<\/li>\n\n\n\n<li>We recommend asking your insurance provider the following to help determine your benefits:\n<ul class=\"wp-block-list\">\n<li>Does my health insurance plan include mental health benefits?<\/li>\n\n\n\n<li>Do I have a deductible? If so, what is it, and have I met it?<\/li>\n\n\n\n<li>Does my plan limit how many sessions per calendar year I can have? If so, what is the limit?<\/li>\n\n\n\n<li>Do I need written approval from my primary care physician for services to be covered?<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h2 class=\"wp-block-heading\">5. Confidentiality, Privacy and Accessibility<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">All personal health information collected through services is subject to federal and state confidentiality laws, including HIPAA, and is governed by our <strong>Privacy Policy<\/strong>. You acknowledge that, while we use secure platforms, electronic transmission carries inherent risks.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Clients have the right to participate in the development of their treatment plan, to be treated with respect, and to request reasonable ADA accommodations. Requests may be made via email or phone.<\/p>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h2 class=\"wp-block-heading\">6. Record Retention &amp; Access<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Client records are retained in accordance with applicable state and federal law. Clients may request copies of, or amendments to, their clinical records in writing.<\/p>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h2 class=\"wp-block-heading\">7. Website Content and Accuracy<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Website content is provided &#8220;as-is.&#8221; While we strive for accuracy, we make no representation that materials are complete, current, or applicable to every individual.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">We disclaim liability for reliance on website information outside of an established provider-client relationship.<\/p>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h2 class=\"wp-block-heading\">8. All website content is for educational purposes only.<\/li>\n\n\n\n<li>Your sole remedy for dissatisfaction is to discontinue use of services.<\/li>\n<\/ul>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h2 class=\"wp-block-heading\">10. Indemnification<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">You agree to indemnify and hold harmless the Practice, its providers, employees, and contractors from claims, damages, or expenses resulting from:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Misuse of services or the website.<\/li>\n\n\n\n<li>Misrepresentation of your location, eligibility, or health information.<\/li>\n\n\n\n<li>Violation of these Terms.<\/li>\n<\/ul>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h2 class=\"wp-block-heading\">11. Termination<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">The Practice may suspend or terminate services for non-payment, provision of false, incomplete, or misleading information, or if continued treatment is determined to be unsafe, unlawful, or unethical. Except in cases of immediate risk or mandated reporting, we will provide the client with reasonable written notice (generally not less than 30 days) of termination, explicit recommendations for ongoing care, and referral resources to alternative providers. All terminations will be fully documented. Termination does not relieve either party of obligations for services rendered prior to termination.<\/p>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h2 class=\"wp-block-heading\">12. Dispute Resolution<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">If a dispute arises under this Agreement and cannot be resolved by mutual consent, the parties will attempt mediation in good faith for up to thirty (30) days after notice. If not resolved, disputes shall be resolved by confidential, binding arbitration under AAA rules in Hillsborough County, Florida. By agreeing to these Terms, both parties waive any right to a jury trial or to bring or participate in a class or collective action. These provisions do not restrict a client&#8217;s right to file complaints with relevant professional licensing boards. Parties are encouraged to seek independent legal advice before agreeing to arbitration.<\/p>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h2 class=\"wp-block-heading\">13. Disclaimer and Scope of Information<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Nothing on this website is intended to diagnose, treat, cure, heal, or prevent any medical or mental health condition.
